Students applying for admission must submit a completed application and all supporting documents to receive consideration for admission. Incomplete admission files will not be reviewed.

FRESHMAN APPLICATION FORMS

Application for Admission

Freshman applicants may apply online, through the Common Application or by paper application.

All applicants must submit a non-refundable $55 application fee, which can be paid online or  by check (addressed to the Office of Admissions, 4588 Parkview Place, St. Louis, MO 63110).

Transcript Request

Freshman applicants must submit official transcripts for all coursework through junior year and a mid-year report for current courses and a list of spring semester coursework.

Recommendation Forms

Freshman applicants must submit three formal recommendations - one Character and General Academic Recommendation, one Science Recommendation, and one Mathematics Recommendation.

Enrollment Forms

After receiving your acceptance letter, you should complete your enrollment process right away. All students must submit an Intent to Enroll form and a $300 enrollment deposit, which can be paid online or by check (addressed to the Office of Admissions, 4588 Parkview Place, St. Louis, MO 63110).

Students applying under Early Decision must also submit an Early Decision Agreement.

Health Forms

All students must submit a Health Records form that includes an immunization history and medical evaluation signed by the student’s primary-care physician as well as an Emergency Contact form. All health information is kept strictly confidential under the Family Educational Rights and Privacy Act.

UNDERGRADUATE TRANSFER APPLICATION FORMS

PROFESSIONAL (P1) APPLICATION FORMS

Application for Admission

Professional applicants complete the application administered by the Pharmacy College Application Service (PharmCAS). The PharmCAS application becomes available on July 15.

All applicants must submit a non-refundable $55 application fee, which can be paid online or by check (addressed to the Office of Admissions, 4588 Parkview Place, St. Louis, MO 63110).

Supplemental Materials

After completing the application via PharmCAS, professional applicants must complete the Statement of Commitment and Technical Standards. These materials should be submitted to the Office of Admissions (4588 Parkview Place, St. Louis, MO 63110).

Enrollment Forms

When you receive your acceptance into the professional program, you will also receive an Intent to Enroll form that must be returned within 10 days of acceptance to secure your place in the professional class.

All students must submit a $300 enrollment deposit, which can be paid online or by check (addressed to the Office of Admissions, 4588 Parkview Place, St. Louis, MO 63110).
